Wellness Works Therapy has a full Intake Department dedicated to helping answer all your questions. We strive to make the intake process as efficient and effective as possible. We pride ourselves on creating the best patient experience possible through amazing customer service and successful therapy services.

Our intake process always begins with a verification of benefits so that each patient is aware of what their insurance covers prior to your first visit. We always aim to be upfront, honest, and save the patient as much stress as possible.

Intake Check List:

Below is the list of items that we need in order to get you scheduled. All of these items are necessary so that we can do our due diligence in advance.

  • Patient Questionnaire (15 minute Phone Interview or fill out Intake Form) – This is where we get all of your information including contact info, insurance info, diagnosis, doctor information etc. (HMO’s need to provide us with the authorization from your Medical Group or IPA.) We will then process your verification of benefits after we have this information.

 

  • Prescription (must be within 30 days) – The State of California requires all licensed therapists to work with the care of a physician, which requires us to have a prescription/referral script from your doctor. It is essential that it is faxed to us at (818) 760-0086 ASAP so we can verify your benefits based on the doctor’s diagnosis. Prescriptions must state the following:
    1. Discipline (Occupational, Physical, or Speech Therapy)
    2. “Evaluate and Treat”
    3. Diagnosis (Speech Disorder, Fine Motor In-coordinator,  CVA – Post Stroke, Muscle Weakness, Cerebral Palsy, Down Syndrome etc.)

  • Patient Paperwork and Forms – To better serve our patients, we ask that all paperwork is filled out and submitted through e-mail, fax, or mail. It is essential that we receive the paperwork 48 hours prior to your first appointment. This allows our office to review all your information prior to your appointment and gives the therapists the opportunity to prepare for your first visit. When we are not able to receive the information in advance, our therapists must review your paperwork during the evaluation, which prevents our patients from getting the most out of their therapists. Our Intake Department will e-mail you all the forms in advance once they receive your intake with your e-mail address.
  • Patient Previous Medical Reports It is essential that you or your doctor send us all relevant doctor reports, pertinent diagnostic tests, barium swallow study (all swallow patients), hospital discharge reports, surgery reports, x-rays etc. These reports help our therapists prepare for your evaluations and are a necessity in order to deliver a great product.

**All hand patients, swallowing patients, and voice patients cannot be seen without all relevant reports.**
